Park Hill Starts their Culture Change Journey!

Senior Housing Options was contacted about the possibility of participating in a new CCCC pilot program funded by The Colorado Health Foundation to provide technical assistance program to Assisted Livings with a high concentration of Medicaid residents to promote culture change practices. The pilot includes: having regular team meetings including staff and resident members, educational sessions with staff on topics pertinent to the project, collaborative workshops with the other Assisted Living participants, completing a survey of residents called The Well being Assessment Tool, attending educational forums, and getting the pleasure of working with Carmen Bowman, the lead consultant and resident-centered care guru and Penny Cook, Executive Director of the CCCC. This project will end in November of 2014. SHO is excited to be a part of this dynamic process and continue it’s work in person-centered care.

Residents and staff alike are excited about this project and had a lot of new ideas to share. The group shared the experience of a learning circle on their first meeting! Resident and staff buy-in into this process is key to its’ success.
