NEW ONLINE PREREQUISITE TRAINING TO HELP YOU PASS THE QMAP CLASS

All students have the opportunity to take an online course BEFORE attending one of our monthly QMAP classes. This is voluntary, but highly recommended!   See below for more details.

As a service to the community of older-adult care providers, Senior Housing Options offers Qualified Medication Administration Personnel (QMAP) classes on a monthly basis. Sessions are taught by Amy Yount, LPN and are usually scheduled on the fourth Thursday of each month from 9 a.m. to 5 p.m. Testing is held on the next day, Friday at 8 a.m. Participants should allow six to eight hours for the written and practical exams and additional instruction at this time.

Classes are limited to 15 students who will be registered on a first-come, first-served basis when payment and pre-registration form are received. See below for class availability.

Tuition for the entire course is $55 and for testing or retesting only $30, due ASAP. Corporate check or money order (no personal checks will be accepted) can be sent to ATTN: QMAP, Senior Housing Options, 1510 17th St. Denver, Co. 80202-1202. Corporate checks and money orders should be made out to C.D.P.H.E. (Colorado Dept. of Public Health & Environment). Please make sure that you print the pre-registration form and mail it with your money order. Money orders without the registration form will be returned. After receipt of payment, SHO will only contact you IF there is a problem with the form, payment, or class date. Please make sure you complete the pre-registration form and all required fields, including email address.

All classes are located in Denver. Registrants will be sent direction to the email address indicated in their registration materials. Information will be EMAILED to you 2 weeks prior to the class starting. If you are unable to attend a class you have registered for, please call 303-595-4464 x 10 by 3 p.m. the Friday of the week before the class so that we can arrange for another student to take your place. If you do not call by the Friday of the week before the class and don’t show you will forfeit your registration/class fee. NO EXCEPTIONS.

 How to Enroll in a Class:

1. Before enrolling in a QMAP class, you are encouraged to take a voluntary online training course that will help you prepare for and pass the QMAP class (see below for more information). 

2.  Print out Pre-Registration Form

3.  Complete Pre-Registration Form

4.  Mail Pre-registration Form and $55.00 (paid by Moneyorder made out to C.D.P.H.E.)to:

5.  When the registration is received and the student accepted, the student will receive a confirmation, class location/information and study guides before the scheduled class date.  Information will be EMAILED to you 2 weeks prior to the class starting. Study guides can be found below.
